If you have DENTAL insurance, please bring your DENTAL insurance card/information to your first appointment. Without your vital DENTAL insurance information, we are unable to process dental insurance claims. Thank you.
What To Expect
Being well-prepared for your appointment will ensure that Dr. Lau and Dr. Merry have all of the necessary information to provide the best possible care for you. It may also help relieve any unnecessary anxiety you may feel. If you have not yet completed our new patient forms, please arrive 5-10 minutes prior to your appointment time to complete them in office.
